Distal Radius Fracture Recovery: Surgery Versus Casting and the Timing of Rehabilitation — Findings From Two Systematic Reviews and Meta-Analyses
After a wrist fracture, what moves function more: the fixation decision made in the fracture clinic, or how soon rehabilitation starts? Two pooled analyses, reported side by side.

Physical therapists and physical therapist assistants who treat adults after a distal radius fracture in outpatient orthopedics, hand therapy, or post-acute settings, and clinicians who field patient questions about whether surgery or a cast produces the better wrist. Physical therapist assistants work under the direction of the supervising physical therapist; evaluation, diagnosis, and plan-of-care decisions remain with the therapist.
Physical therapists and physical therapist assistants; also useful to occupational therapists and athletic trainers who manage upper-limb fracture rehabilitation.

After completing this course, you will be able to:
- State what the pooled comparison of surgical and conservative management of distal radius fractures reported for wrist motion, grip strength, patient-rated scores, radiographic alignment, and complications.
- Describe what early rehabilitation after volar plate fixation changed, and did not change, across upper limb function, wrist-specific function, pain, and complication rates.
- Distinguish which of the two decisions — the fixation choice or the timing of rehabilitation — the pooled evidence connects to therapist-influenced functional outcomes, and explain what each review does and does not support telling a patient.
